Social work support / Whānau Tautoko is a wrap around, strengths based approach to supporting whānau to address issues within their whānau and home to improve their overall holistic wellbeing. A holistic plan is mapped out alongside the whānau with achievable goals to strengthen the client and whānau to see beyond the horizon to bring dreams closer, taking whānau out of change and into living. Goals are designed to use the client and whānau strengths to address issues and boundaries that are stagnating their ability to reach for their dreams and aspirations.
Targeted social work support and multidisciplinary services to whanau of at risk children and youth. Whānau take part in an assessment to identify relevant goals for positive and agreed outcomes. Plans developed with whānau to achieve identified outcomes for the young people, siblings and members of the whānau.
